This is a new Altech 616.3500.703 3-pedal foot switch made by Bernstein Classic foot switches.
This switch had a cord and stickers added to it to be used on a special fabric cutting machine but was never put in service.
This foot switch comes with a 4 foot cord that has an AMP AMP 208716-1 0440 7-conductor Round Male Plug.

This is made of heavy duty aluminum.
The pedals on each side have 1-N.O. and 1-N.C. contact each (single speed). These pedals when fully depressed also activate a limit switch that has 2 N.C. contacts. This requires considerably more pressure, and once fully depressed they lock in the fully depressed position until the button on the end is pressed to release them.
The center pedal has 1-N.O. and 1-N.C. that switch in the first stage and if the pedal is depressed all the way (requiring considerably more pressure) it has another set of contacts (1-N.O. and 1-N.C) (two speed).
